Use this form if you do not have an active eSantaFe login. This includes future students, former students, parents, and others contacting us on behalf of a student.

Responses to this form will not include information protected by FERPA, even when a FERPA release is on file, because we cannot verify identity through this form. For anything involving a student's record, the student should submit the inquiry themselves.
